Click on Create an Account; this brings you to the Client Portal hosted by MindFieldsHealth.com partner, AdvancedMD.  Complete the required information (note: there are line entries for 'insurance information', however this is a 'place-holder' and is not required to complete your Account Registration).  After receiving the completed Account Registration, a MIndFields Health review specialist will contact you promptly and present recommendations customized to your goals and requirements.

What should I expect after creating my account?

Prior to your Tele-visit, an review specialist and provider will review your chart and determine the level of care; this process involves a number of considerations, including: length of time, prescription requirements, if any, and other mitigating circumstances; charges will be quoted prior to services, and revised if additional [or fewer] factors were addressed during  your Tele-care encounter.  For a limited time, MindFieldsHealth.com is offering the first Therapy session at $95.

How does the Credit Card Process Work?

After your visit,  the provider will review the services, enter a detailed chart note, and input a charge to the credit card on file.  The charge reflects a composite of evaluation, management, and complexity of the presenting condition(s).  A detailed receipt (superbill) is sent to you. For those who require maximum privacy, the receipt is annotated to your specific direction.
